HIPAA Policy – CONFIDENTIALITY OF PATIENT INFORMATION

The Health Insurance Portability and Accountability Act (HIPAA) mandates that health care providers maintain the confidentiality of patients’ personal health information, which includes both medical information and individually identifiable information about any patient such as address, phone number, or Social Security number, that could reveal the person’s identity. OnPoint Urgent Care prohibits the unauthorized disclosure of such information except as required by law.

Disclosing confidential patient information to other employees except when needed to provide care is prohibited. Practice physical, electronic, and procedural safeguards are in place that comply with regulatory standards. You are expected to maintain these safeguards when working with patient information. Because of the importance of patient privacy, failure to comply with this policy may result in disciplinary action, up to and including discharge.

Upon hire, you are expected to sign a Confidentiality Agreement that covers both patient and practice information.
